When creating a PPQ Form 579 Phytosanitary Certificate for Reexport, the Foreign Certification Information page captures the certificate details entered in the Application Step by Step submission process.
To complete the Foreign Certification Information page fields of the Application Step by Step, complete the following steps:
| 1. | Select the name of the country of origin from the drop-down list and click the Add button to add the Country Of Origin.
NOTE: At least one Country of Origin needs to be selected. |
| 2. | The Foreign Certificate(s) Number(s) field displays the original Phytosanitary Certificate Number. If there are no certificate numbers, enter None or leave blank. |
| 3. | In the Foreign Certificate(s) Are field, select the appropriate checkbox to indicate whether the Phytosanitary Certificate is an original or a certified true copy. |
| 4. | In the Commodities Are field, select the appropriate checkbox to indicate whether the product is packed, repacked, in original container, or in a new container. |
