1.To enter the export & shipping information for your certificate, complete the following steps:

 

 

U.S. Port of Departure

Select the name of the port of embarkation/border crossing from the drop-down list.
If the Port being used is not shown in the drop-down list, then select “Other” and enter the Details in the field labeled “If Other, More Details."  The Details are required if “Other” is chosen.
This is a required field.

Estimated Date of Shipment

This is the estimated date that the commodities will be shipped.
Enter the date in mm/dd/yyyy format or click the calendar icon to select the appropriate date.
This field will not display on the Certificate.
This is a required field.

Means of Transport

Select either Airplane, Ship, Train, or Truck as the means of transport from the drop-down list.
This is a required field.

Means of Transport Identification

Enter any identification or markings for the Means of Transport.
This is an optional field.

Number of Containers

Enter the number of containers being exported or shipped.
This is an optional field.

Container Description

Select either Boxes, Cages, Stalls, Truck, or Other as the container description from the drop-down list.
If the Container Description is not shown in the drop-down list, then select “Other” and enter the Details in the field labeled “If Other, More Details."
This is an optional field.

Identification/Seal Numbers

Enter the identification/seal number and click the Add button.

   RESULT:  The identification/seal number displays in the table.  You can delete an identification/seal number by selecting the corresponding checkbox and clicking the Delete button.  You can also use the Select All link to delete all the identification/seal numbers shown in the table.

This is an optional field.

 

2.When you have completed entering your information, click the Next button to proceed.  You can also click the Save as Work in Progress and return to the certificate at a later time.

 

HELPFUL HINT: You may enter certificate information in any order by selecting the links at the left of the screen; for example, you can enter Certificate Content information and then General information.  In this Help file, data entry into the certificate will be explained sequentially as if using the Next button.  At the bottom of each page the Previous, Save as Work in Progress, and Next buttons are available.

 

The type of Certificate (Certificate Content) and the unique Certificate Tracking Number is always displayed at the top of each Certificate screen.
