General Page Overview

The General page field descriptions are as follows:

Applicant Name

•	Displays the company name which appears on the applications/certificates.

Applicant Address

•	Displays the company address and phone number which appears on the applications/certificates.

Exporter Name

•	From the drop-down list, select the name of the exporter for this application. If the exporter’s name does not appear on the list, click the Add button beside the Exporter name drop-down list and the Add Exporter page appears.

For ACO’s, the Exporter Name list includes names from the applicant as well as the ACO’s preferred duty station. When ACO’s click the Add button to add a new exporter, by default the exporter is saved to the applicant’s account. ACO’s can save the exporter to their list by checking the Save Exporter to Your Duty Station List box.

Consignee Name

•	From the drop-down list, select the name of the consignee for this application. If the consignee’s name does not appear on the list, click the Add button beside the Consignee name drop-down list and the Add Consignee page appears.

For ACO’s, the Consignee Name list includes names from the applicant as well as the ACO’s preferred duty station. When ACO’s click the Add button to add a new Consignee, by default the consignee is saved to the applicant’s account. ACO’s can save the consignee to their list by checking the Save Consignee to Your Duty Station List box.

NOTE: You can edit existing exporters and consignees through the Manage Template Data link. Refer to Manage Exporters and Manage Consignees Help topics for details.

Requested Issuing Duty Station

•	From the drop-down list, select the duty station that you wish to perform the inspection of the commodity(ies) of this application. Check with your local inspector for Duty Station information.

Reference Information

•	Data used to identify or associate applications.

•	This is an optional field.

Import Permit Number

•	The import number from the foreign government.

NOTE: Use a comma to separate multiple import permit numbers.

•	This is an optional field.

Comments

•	Any instructions or other information for inspector review. General comments are displayed on 572 applications if the field contains comments; however comments are not displayed on 577 or 579 applications.
